   Examples of coverage-per state. Oklahoma is not covered.
 *(For Example): Some states are covered but the visits are six months apart. 

That sounds like the government paying the clinic for the service but the clinic not really giving any beneficial treatments. (You run into a lot of that type arrangement when dealing with government subsidized services.)
